  • I understand that yoga is not a medical procedure and that the teacher/therapist will not be diagnosing any medical concerns that I may have.
  • I understand that yoga is comprised of techniques for facilitating wholeness, body awareness and awareness of self. These exercises may develop strength, flexibility, endurance, stamina, relaxation, concentration and breath awareness and may involve the use of equipment such as mats, belts, pillows, blankets, ropes, blocks, chairs, etc.
  • I recognize and agree that I am fully responsible for my health, safety and wellbeing and that I am participating in this activity voluntarily.
  • I have reviewed the ParQ available at http://eparmedx.com/wp-content/uploads/2013/03/PARQplusJan2015.pdf with respect to my personal health and believe myself to be sufficiently fit to participate . Even so, I will inform the teacher/therapist of any positions, movements or activities that I do not think I can safely perform. I will practice gently and will not partake in such positions, movements or activities I think are likely to cause me injury in the class or when I practice at home.
  • I agree that my participation may be dependent upon provision of health information, and possibly a release form from my medical provider and/or a private assessment by the yoga teacher/therapist if deemed appropriate by the teacher/therapist.
  • I further acknowledge that I agree to hold Lonnie DeSorcy (the yoga teacher/therapist) and any owner of facilities or equipment used in yoga instruction, as well as referring or consulting medical provider or his/her staff harmless from any and all responsibility for any injury or discomfort that I may sustain during, or as a result of, attendance and participation in yoga sessions or any errors of omissions I may have made.
  • This release shall be binding upon my heirs, executors and administrators.
